Massage and other treatments are charged separately, and River Kwai Village does packages with an overnight stay if you'd rather make a proper trip of it.
Two things to set expectations. Front desk service comes up repeatedly as cold and short on information, so don't expect hand-holding. And the fish spa in the name has left more than one visitor staring at an empty pool. Come for the water, treat the rest as a bonus.

It was a super experience! There are 15 baths with milk, wine, and coffee of different temperatures. It's just magical! the entrance fee is 650 baht, you can get a massage for an additional price. They give you bathing suits, you can't wear your own swimsuit.

There are two ways to access the area: by boat or via a local road, which, though unpaved, is still accessible. I highly recommend coming by car to ensure you have plenty of time to relax and enjoy your visit. The facilities are very clean, and the service is excellent!
Disagree with the good reviews. First when I am first on the lobby, she said only 650baht with no smile and no other more information was told. Second the cleanliness is badly ignored. Third, no fish in the fish pool. No one can explain that only with the answer “there’s few….” But not only one client said there’s no fish at all. Fourth, all the staff has no smiles . Total no service at all. But the scenes are good
