Koh Phangan to Phuket
A late-morning boat off the Gulf, then across the peninsula to the Andaman
Koh Phangan is in the Gulf of Thailand and Phuket is on the Andaman Sea, so no boat goes between them. Every ticket is a boat to Donsak pier on the mainland and then about 290 km of road across the peninsula, or a boat to Koh Samui and a flight. Going this way, the boats leave late in the morning and you reach Phuket in the evening. Lomprayah's 11:00 from Thong Sala reaches its office in Wichit, west of Phuket Town, at 18:30 for ฿1,400, and agencies sell the same boat with a van that drops at Phuket airport at about 18:00 or at your hotel in Patong, Kata or Karon by about 20:00.
Check live Koh Phangan → Phuket times and seats on 12Go
Songserm's 12:40 is the last daytime departure. If you're flying home from Phuket, I'd fly from Samui instead, because the overland day only gets you to the airport in time for a late-night flight.

The short answer
Take Lomprayah's 11:00 boat and coach from Thong Sala, in Phuket at 18:30 for ฿1,400, or a resold version with a van to your Phuket hotel or the airport for about ฿1,350–1,600. The cheapest daytime tickets leave Thong Sala between 05:00 and 11:30 for about ฿600–1,300, and the last daytime departure is Songserm's 12:40. To catch a flight from Phuket the same day, take a boat to Samui airport and fly. There's no direct ferry.

It sails to Surat Thani overnight and connects with a bus that reaches Phuket Bus Terminal 2 at about 13:00, for about ฿970–1,180.

Lomprayah's Boat and Coach
Lomprayah's 11:00 from Thong Sala is the ticket I'd book. The catamaran calls at Samui on the way to Donsak pier, a coach takes over for the drive across the peninsula, and it ends at Lomprayah's office in Wichit, just west of Phuket Town, at 18:30. The ticket is ฿1,400, or ฿1,600 with a 10:00 pickup from your Phangan hotel, which covers the whole island except Thong Nai Pan. From Haad Rin the songthaew to Thong Sala takes 20 to 30 minutes, and from Thong Nai Pan 45 to 60, so the pickup saves an early start.
There's one Lomprayah departure a day in this direction, so book a few days ahead from December to March and for the days after a Full Moon Party. From the Wichit office you'll still need a taxi to your beach, which is why the van versions below are the better buy if you know where you're staying.
Getting Dropped at Your Hotel or the Airport
Several agencies resell the same 11:00 boat with their own van from Donsak. On mid-November and mid-December weekdays 12Go had drops at Phuket airport at about 18:00 to 18:45, Phuket Town at 18:30 to 19:15, Bang Tao and Surin at 18:30 to 19:00, Patong and Kamala at 19:30, and Kata and Karon at about 19:45 to 20:00, for about ฿1,350–1,600. The vans drop in order around the island, so the south-coast beaches come last.
The 11:30 tickets from Thong Sala ride a ferry instead of the catamaran and still reach Phuket Town by about 19:15 and Patong by about 19:30, for about ฿1,000–1,300. One company, TransBaanTon, collects you from your Phangan hotel at 10:00 and drops at Phuket beach hotels between about 18:00 and 20:00 for about ฿1,080–1,280. Check that your ticket names your beach, not just Phuket.
The Cheaper Bus and Ferry Tickets
The bus-and-car-ferry tickets take about 2.5 hours to cross to Donsak against an hour on the catamaran, and they leave earlier. 12Go's timetable lists Raja's buses and car ferries from Thong Sala at 05:00, 08:00 and 11:00, reaching Phuket at about 16:00 to 18:30, and Phantip's 08:30 and 11:30, reaching Phuket Bus Terminal 2, north of Phuket Town, at about 17:00 and 18:30, all from about ฿600. Its live board for mid-November showed fewer of them and higher fares, Phantip's 08:30 at about ฿1,200 and a Raja 07:00 with a drop at Phuket airport at about 18:00 for about ฿1,300, so expect anything from about ฿600 to ฿1,300 for your date.
Bus Terminal 2 is a bus station, not the airport, so from there it's a taxi to your beach or the airport.

Songserm's 12:40 and the Night Boat
If you'd like a slower morning, Songserm's 12:40 speedboat from Thong Sala connects with a van that reaches Phuket Bus Terminal 2 at about 19:00, for about ฿1,600. Resold versions go on to the airport at about 19:30 and to Patong, Kata and Karon at about 20:30. It's the last departure that reaches Phuket the same day.
After that, the night boat leaves Thong Sala at 22:00 with mattresses on deck, reaches Surat Thani early in the morning, and a bus takes you on to Phuket Bus Terminal 2 at about 13:00, for about ฿970–1,180 on one ticket. It saves a night's room, though you'll have slept on a boat.
Flying via Koh Samui
Koh Phangan has no airport, so flying means a boat to Samui first. Lomprayah sells a ฿500 ticket from Thong Sala to Samui airport, boat to Bangrak and a van, at 07:30, 10:45, 12:30, 13:40 and roughly hourly from 15:00 to 18:00, arriving about an hour after you sail. Bangkok Airways flies Samui to Phuket nonstop at 08:15, 11:10, 12:30, 15:05, 15:50 and 18:15 on a mid-November weekday, in about an hour, for about ฿3,500–4,000, with extra flights at 09:20 and 19:00 by mid-December.
Give the flight 2.5 hours from the boat's arrival at Bangrak. That makes the 07:30 boat right for the 11:10 flight, at Phuket airport by 12:10, the 10:45 boat right for the 15:05, and the 13:40 or 15:00 boat right for the 18:15. It costs about three times as much as the boat and bus, but you're in Phuket by lunchtime. Our Koh Phangan to Koh Samui and Koh Samui to Phuket guides have every boat and flight.
Can You Make a Flight From Phuket the Same Day?
Overland, only a late one. The earliest drop at Phuket airport is about 18:00, you need to be there two hours before an international flight, and the boat and van run late often enough that I wouldn't book anything before about 22:00. In the rough season from October to December, when boats can be canceled, don't plan on it at all.
The safer ways are to fly from Samui, where the 07:30 boat and the 11:10 flight have you at Phuket airport by 12:10, or to cross the day before and spend a night in Phuket. Our where to stay in Phuket guide covers the beaches nearest the airport.
Coming the Other Way, Phuket to Koh Phangan
Going east the day starts earlier. Lomprayah's coach leaves Phuket at 09:30 and lands at Thong Sala at 16:30. Our Phuket to Koh Phangan guide has every ticket, the hotel and airport pickups, and the flights to Samui that connect.

Frequently Asked Questions
Common questions about Koh Phangan to Phuket.
Take Lomprayah's 11:00 catamaran and coach from Thong Sala, which reaches Phuket at 18:30 for ฿1,400, or a resold version with a van to your hotel or the airport for about ฿1,350–1,600. Songserm's 12:40 is the last daytime departure, the 22:00 night boat arrives around 13:00, or take a boat to Samui and fly.
Not directly. Koh Phangan is in the Gulf and Phuket is on the Andaman coast, so every ticket is a boat to Donsak or Surat Thani plus a bus or van across the peninsula, or a boat to Koh Samui plus a flight.
About 7.5 hours on Lomprayah (11:00 → 18:30), up to 9 hours with a hotel drop at Kata or Karon, under 5 hours flying from Samui on the 07:30 boat and the 11:10 flight, and about 15 hours on the night boat.
฿1,400 on Lomprayah's boat and coach, about ฿1,350–1,600 with a van to your hotel or the airport, about ฿600–1,300 on the bus and car ferry tickets, about ฿1,600 on Songserm and about ฿970–1,180 on the night boat. Flying costs ฿500 for the boat to Samui airport plus about ฿3,500–4,000 for the flight.
Songserm's 12:40 from Thong Sala is the last that reaches Phuket the same day, at about 19:00. After that, the night boat leaves at 22:00 and reaches Phuket at about 13:00 the next day.
Not from Koh Phangan, which has no airport. Take Lomprayah's ฿500 boat-and-van ticket to Samui airport and fly Bangkok Airways to Phuket, six flights a day on a November weekday, about an hour, from about ฿3,500.
Only a late one overland. The earliest airport drop is about 18:00, so I wouldn't book a flight before about 22:00. The 07:30 boat to Samui airport and the 11:10 flight get you to Phuket airport by 12:10, which is the safer way.
About 290 km by road from Donsak pier to Phuket, after the boat from Thong Sala to the mainland. The flight from Samui to Phuket takes about an hour.
