Koh Samui to Chiang Mai
One morning nonstop, and it's the cheap direction
Bangkok Airways flies Koh Samui to Chiang Mai nonstop once a day, leaving Samui at 08:50 and landing at 10:40, and that's the one I'd book. In this direction it's also the cheap option. 12Go showed it at about ฿4,300 for November and December dates and about ฿7,000 for a March date, well under half what the same airline charges going south. Flying from Surat Thani instead means a bus and boat to the mainland and Thai AirAsia's 19:55 to Chiang Mai, about ฿3,500, which saves little once you add the ฿850 boat ticket and loses the whole day.

One-stop flights via Bangkok cost about twice the nonstop. There's no train or bus all the way. Overland it's the boat to Surat Thani, the sleeper to Bangkok and the day train north, about 30 hours.

Key Facts
One nonstop a day, 08:50 to 10:40
Bangkok Airways flies it in 1 hour 50 with a checked bag included. It's the only nonstop, so book it first and plan the island morning around it.
About ฿4,300 for November and December dates
12Go showed ฿4,346 for mid-November and mid-December dates and about ฿7,000 for a March date. Going south, the same route was about ฿10,000.
Leave your hotel by about 07:30
Samui airport is 10–20 minutes from Chaweng and Bophut and longer from Lamai and the west coast. A 08:50 departure means being at the airport by about 07:50.
Via Surat Thani rarely pays this way
Thai AirAsia flies Surat Thani to Chiang Mai at 10:30 and 19:55 for about ฿3,500, but the 10:30 needs a night on the mainland and the 19:55 needs a boat at breakfast time.
By train it's about 30 hours
The 19:31 sleeper from Surat Thani reaches Bangkok at 06:10, and day train No. 7 leaves the same station at 07:30 and reaches Chiang Mai at 17:50.

The 08:50 Nonstop
One flight a day, and the cheaper direction
Bangkok Airways is the only airline flying Samui to Chiang Mai without a stop, once a day. The flight leaves at 08:50 and lands at 10:40, early enough that you're in the Old City for lunch. Thai Airways sells seats on the same flight under its own number, so it isn't a second option. A 20 kg checked bag is included.
Fares run lower going north. For dates in mid-November and mid-December, 12Go showed ฿4,346, and for a date in March about ฿7,000, where the flight south sat at about ฿10,000 on every winter date I checked. If you're doing both legs, that's worth knowing before you pick a direction for the trip.
Flying From Surat Thani Instead
Thai AirAsia flies Surat Thani to Chiang Mai nonstop at 10:30 and 19:55, 1 hour 50 each, and 12Go showed about ฿3,500 for a November date. The boat is what makes it awkward. The bus-and-boat tickets from Samui to Surat Thani airport take 3 to 4 hours, so the 10:30 means crossing the day before and sleeping in Surat Thani, and for the 19:55 you'd take Lomprayah's 08:30 from Nathon, at the airport by about 11:30, then wait most of the afternoon. With the ฿850 boat ticket it costs about the same as the nonstop on the dates I checked, so I'd only do it if the nonstop is sold out or you're already on the mainland. Our Surat Thani to Koh Samui guide has the boats to the airport.

One-Stop Flights via Bangkok
Bangkok Airways sells Samui to Chiang Mai with a change at Suvarnabhumi from about ฿8,300 on 12Go, leaving Samui from 07:00 to mid-afternoon and taking about 4 to 5 hours. They're worth it only when the 08:50 is full or you need an afternoon departure. Skip the overnight connections that leave Samui in the evening and land the next morning.
Koh Samui to Chiang Mai by Train
The sleeper to Bangkok and the day train north
There's no train from Samui, so the first leg is a boat-and-bus ticket to Surat Thani's station at Phun Phin, sold at every pier office for about ฿500–900 and in time for the evening trains if you leave the island by late morning. Express No. 86 leaves Phun Phin at 19:31 and reaches Krung Thep Aphiwat in Bangkok at 06:10 (2nd-class air-con berth about ฿755 upper, ฿825 lower). Special Express No. 7 leaves the same station at 07:30 and reaches Chiang Mai at 17:50 (2nd-class air-con seat ฿558). That's about 30 hours from a late-morning boat for about ฿1,800–2,300.
The 1 hour 20 at Krung Thep Aphiwat is fine when No. 86 runs on time, but it comes up from Nakhon Si Thammarat and can run late. For a safer plan, spend the day in Bangkok and take the No. 9 sleeper north at 18:40, into Chiang Mai at about 07:15. Our Bangkok to Chiang Mai guide covers every northbound train, and seats on both legs go on sale 90 days ahead.
Chiang Mai Airport (CNX) → Old City / Nimman
Getting in from Chiang Mai airport
Chiang Mai's airport is barely out of town, ten minutes to the Old City. The taxi counter in arrivals sells a fixed ฿160 ride, Grab runs ฿100–200 from the parking lot, and a car pre-booked on 12Go (from about ฿450, more often ฿750–1,000) is the fixed-price option.
